The Senior Community Care Program (SCC)
is a vital part of the continuum of care provided by Dominican Sisters Family Health Service. The SCC provides non-medical support services including case management, subsidized house cleaning, socialization, recreation programs and friendly visiting to the growing population of frail, elderly Westchester County residents. These services enable the elderly to remain independent and safely in their own homes and communities for as long as possible. this program works with local churches and synagogues.

ELIGIBILITY:
Pastoral Care is an essential component of care – it touches the heart of the Agency’s philosophy and the healing ministry of Jesus. This unique service is available at all locations to those in need as a part of holistic health care. Trained counselors assist patients, families and staff during times of crisis to cope spiritually, physically and emotionally with terminal and chronic illness. All are helped to find inner strength in the face of illness, isolation, death and grief. Care includes Bereavement support following the death of a loved one. Collaboration with and referrals to clergy of all faiths are part of this program.
These vital services are not reimbursed and are supported through fund raising activities and contributions.
"Sickness creates for the sick a threefold crisis of Faith, Hope and Love.
…Healing enables the sick and all who participate in their sickness to resolve the crisis…"
